Case Management Services

We provide in-depth targeted case management to individuals in Johnson County, KS; Douglas County KS, and Jefferson County, KS.    We are an independent Targeted Case Management Agency.

​Our staff includes over 70 years of case management experience serving children and adults in the community, including special education experience at all levels.  We provide supports for the IDD, LD, BD community at any age level.  We are private pay, but do take KanCare for IDD clients who are qualified for Title 19.  Services provided through case management are:

  • Person Centered Support Planning
  • Behavior Support Planning
  • IEP Attendance
  • Employment service referrals
  • Employment observations and recommendations
  • Community event referrals
  • Community support referrals
  • Home visits
  • LD Baseline testing

What is HCBS?

​Home and Community Bases Services (HCBS) provides oversight for a system community based supports and services for persons in Kansas with disabilities.    Providers include a variety of private not-for-profit and for-profit organizations as well as governmental entities. These services are provided through six programs.
